Emergency Info Card Generator
When something goes wrong, the people helping you need answers fast: what medications you take, what you are allergic to, who your doctor is and who to call. This free generator turns those details into two printouts. The first is a folding wallet card that sits behind your ID. The second is a full-page fridge sheet, because the fridge is an easy place for paramedics, neighbors and family to find it when someone cannot speak for themselves.
Who it is for
It is made for older adults, anyone who lives alone, and adult children setting things up for a parent. If you manage a parent's medications, fill it in together on a visit, print two copies of each, and keep one yourself. It is also useful for anyone with a long-term condition, a blood thinner, or a serious allergy.
How to use it
- Fill in the form below. Only your name is really needed; leave anything blank you prefer not to share.
- Press "Create my card" to see the wallet card and the fridge sheet.
- Print the wallet card at 100% scale, cut along the dashed line and fold it in the middle.
- Print the fridge sheet and put it on the fridge with a magnet. Update both whenever a prescription changes.
Nothing you type leaves this device. There is no login, nothing is saved, and nothing is sent to a server. Close the page and it is gone. If you would like the same details kept safely on your phone, the TapOkay Emergency Safety Vault (part of TapOkay Pro) stores them encrypted and masked, ready for the people in your circle when it matters.
A good habit is to review the card on the same day each year, such as a birthday, and whenever you leave the hospital or start a new prescription. An out-of-date list can be more confusing than no list at all.
